Influencia de la curación tradicional de las heridas crónicas en los costos de la estancia hospitalaria en los usuarios externos del Hospital General HOSNAG. Creación del servicio de clínica de heridas.

Abstract

The chronic wounds affect economically to the Healthcare System at around the world (Iberolatinoamerica, 2011). Because of this, the research objective is to analyze traditional healing applied to chronic wounds in relation with the external users’ hospital stay costs from the HOSNAG General Hospital. The research approach is quantitative and qualitative where, three data collection methods were applied: 1 - Questionnaire oriented to the internal client, in which was used a transversal descriptive methodology through a modified auto-evaluative questionnaire of 20 questions with dichotomous answers; 2- Historical clinic observation using the above methodology in a retrospective cohort study where, the instrument was the clinic histories between May 2016 to May 2017; and 3- Interviews or phenomenological study oriented to professional expertise in chronic wounds to reinforce this research with the participation of a vascular surgeon, a plastic surgeon and a degree in nursing. As results of the 115 questionnaire applied to the internal client from this hospital, 83% of them applied the traditional healing and 17% the advance healing. On the other hand, the observation of 98 clinic histories from patients with chronic wounds brought out that, the average period hospital stay was 19.17 days. Finally, the results of the professional interviews reinforce the need to have an area specialized in chronic wounds with advanced healing. The advanced cure reflected a saving of $221.25 which meant 43.08% compared to the traditional cure. The total saving generated by the new wound clinic was $ 398 per patient.
